What is a reportable event?

According to the Public Health Service (PHS) a reportable event includes conditions that jeopardize the health or well-being of animals, including natural disasters, accidents, and mechanical failures, resulting in actual harm or death to animals. 

The Office of Laboratory Animal Welfare (OLAW) defines an adverse event as those unexpected incidents that lead to harm or endanger the well-being of animals and humans at a research facility.  

If you are a principal investigator (PI) on a project, you can submit a reportable or adverse event to the IACUC in Kuali Research by going into your protocol and clicking “Reportable Events” at the top of the page.
