- The applicant must have completed, or be in the process of completing, a dental degree program accredited by the Commission on Dental Accreditation of the American Dental Association or the Commission on Dental Accreditation of Canada.
- The applicant must possess or be eligible to obtain an Illinois dental license.
- The applicant must have passed Part I of the National Board Dental Exam, and must also pass Part II before enrollment. Alternatively, the applicant must pass the Integrated National Board Dental Examination before enrollment.
The applicant must complete the Post Graduate Endodontics Resident Application and submit all items as cited in the instructions that follow:

- A non-refundable application fee of $20 is required, and can ONLY be paid with a cashier’s check or money order made payable to SIUE (Southern Illinois University Edwardsville).
- 3 letters of recommendation (if not included in PASS)
- Current curriculum vitae (if not included in PASS)
- Small photograph (2x2)
- If you are not participating through PASS, please submit the following items:
- Post Graduate Endodontics Application
- A non-refundable application fee of $20 is required, and can ONLY be paid with a cashier’s check or money order made payable to SIUE (Southern Illinois University Edwardsville).
- Photocopy of dental school diploma (if available)
- Official transcript forwarded by dental school
- 3 letters of recommendation
- Current curriculum vitae
- Official National Board Scores Part I & Part II or INBDE score
- Small photograph (2x2)
The SIU SDM Post-Graduate Endodontics Resident Application Cycle for the SIU SDM Post Graduate Endodontics Entering Class of 2026 will open in May 2025.
Please note that for the SIU SDM Post Graduate Endodontics class that commences July 1, 2026, the SIU SDM Post-Graduate Endodontics Resident Application Cycle will open in May 2025 and will close on August 1, 2025.

Note for Applicants: We accept Advanced Dental Admission Test (ADAT) scores but do not require them. A candidate’s application will not be negatively affected if an ADAT score is not submitted.
Note for Residents Offered Admission: If your application is accepted and you are admitted into the SIU SDM Graduate Endodontics Program, a $1,000 enrollment deposit is required. Residents must pay a one-time enrollment deposit of $1,000, which will be applied to your tuition expenses. A cashier's check or money order payable to Southern Illinois University Edwardsville (SIUE) is acceptable. Detailed instructions will be provided with the offer of admission. This deposit is non-refundable. Residents will also be required to submit clinical requirements and drug test results to and request a background check from CastleBranch before program commencement for a fee determined by and paid directly to CastleBranch.

SIUE Annual Security and Fire Safety Report
The SIUE Annual Security and Fire Safety Report is available online. The report contains campus safety and security information, crime statistics, fire safety policies, and fire statistics for the previous three calendar years. This report is published in compliance with Federal law, titled the “Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act” and the Higher Education Opportunity Act also known as the “Campus Fire Safety Right to Know.” For those without computer access, a paper copy of the report may be obtained, with a 24-hour notice, from the Office of the Vice Chancellor for Administration, Rendleman Hall, Room 2228, (618) 650 2536.
